AmeriCorps Substance Use Educator and Support
Our AmeriCorps members will increase our reach and scope of substance use education and recovery services by expanding the delivery of youth-focused substance use education in youth-serving community settings. Members will also work closely with our Youth in Action council creating a speaker series. Members will work in clinical departments to assess patients for risky substance use behaviors using standardized screening tools as part of well visits and make referrals/connections to community-based services. Member will provide care coordination to support patients as they transition through phases of substance abuse care. AmeriCorps members will also act as a liaison between treatment teams and assist patients in navigating the myriad of community resources to break down structural inequality which lead to economic marginalization.
